The National Transportation Safety Board (NTSB) issued a final report on the probable cause of a fiery single commercial vehicle crash that caused the catastrophic collapse of a portion of I-95 in Philadelphia in 2023.

The crash occurred at 6:17 a.m. on June 11, 2023, as tanker truck driver Nathaniel Moody, 53, was hauling 8,500 gallons of gasoline north on I-95. Moody lost control on a ramp while traveling at an estimated speed of 44–54 m.p.h. The gas tanker truck struck a wall and overturned, resulting in a massive fire that caused severe structural damage to the overpass, resulting in a partial collapse of I-95. The tanker truck was buried in the rubble under the overpass.

Moody died in the crash.

The final NTSB report found that Moody’s speed and other factors were the probable cause of the crash and overpass collapse.

We determined that the probable cause of the Philadelphia, Pennsylvania, combination vehicle rollover crash, postcrash fire, and subsequent Interstate 95 overpass collapse was the driver’s failure to slow the vehicle as he exited the interstate onto the exit ramp well above the posted advisory speed limit, due to inattention to the roadway potentially associated with fatigue. Contributing to the severity of the postcrash fire was the driver’s failure to secure the vehicle’s manhole cover during his pretrip inspection, which enabled the gasoline that was being transported to quickly enter the environment and spread throughout the crash area.​

As key takeaways following the investigation, the NTSB recommended that motor carriers implement “robust fatigue management programs.” Authorities also suggested that the crash could have been prevented if the truck had been outfitted with an electronic stability control system.

Permanent lanes of northbound I-95 fully reopened in May 2024.
