Police are investigating a fatal accident that took place at a distribution center in Woodburn, Oregon, this week.

The Woodburn Police Department was called to the Winco Distribution Center shortly before noon on Wednesday, June 4.

Forklift operator Christopher Ryan De Jesus, 39, was pronounced dead at the scene.

Investigators believe that De Jesus was still inside of a container when it was moved by a semi truck driver who was apparently unaware of the forklift inside the container. This caused the forklift to fall from the container, leading to the death of De Jesus.

The truck driver remained on the scene and is cooperating with police.

Both De Jesus and the truck driver were third-party contractors, according to KATU.

The incident is under investigation.
